This is how I structure my trading journal and will be doing this after each trade is taken and then closed out.
1). How was I feeling about this set up ?
2). When I entered the trade (Include the date and time) ?
3). Specific details about the trade including the entry price, position size, mental stop loss and profit target, the stop loss and take profit is to be established before taking the trade to ensure good risk to reward.
4). Exit price and my rating out of 5 for exit execution (My main issue is taking profits too early and will be something I will closely monitor with the exit rating).
5). Combined entry and exit rating out of ten include screen shot of trade.

I use Mac Notes, I keep a daily log, whether I trade or not. I will screen cap a ticker I am focused on and will make notes on the screen cap on Photoshop marking my entry and exit and make notes on my thoughts about the trade while I am in the trade. I am NOT a great trader, not incredibly active, and generally a slow learner :( so the effort make for a better learning experience for me at least.
